The Workshop online registration is now closed.  To register, download the and complete Registration Form below and either fax it to +1 202.416.1408 or email it to industry@osa.org.  You can also register on-site at the Anaheim Hilton at the Registration Counter located outside the California Ballroom.

Registration Fees

The registration fee for the workshop includes a day of programming, a networking reception, breakfast, lunch, and refreshment breaks the day of the program.

The member registration rate applies only to employees of OSA Industry Development Associate Member companies. OSA Individual Membership does not apply to this Industry Workshop. To verify if your company is an OSA Industry Member please refer to the Industry Member Directory.

Checks and Money Orders

All checks must be in US dollars and drawn on a US bank.  Make payable to The Optical Society. Please indicate the check number on the registration form.
 

Bank Drafts and Wire Transfers

Please indicate on your registration form which bank was used and when the deposit was made. Also, please notify OSA of the transfer to avoid any lost or unidentified payments. The wire transfer should include the attendee’s name or member’s name and a notation of Executive Forum Registration.  Fax any support documents to Fariba Daneshvaran, The Optical Society, fax number 202-416-1408 or email accrec@osa.org for more additional questions. Please incorporate any bank fees associated with your wire transfer as the bank may deduct their fees from the total sent to us.  The remitter is responsible for these fees.
 

Refund Policy for Registration

A US$ 75 service charge will be assessed for processing refunds. A letter requesting the refund should state the registrant's name and the amount of payment and should be faxed to 202.416.1408. Requests for refunds that are received by 7 MARCH 2016, will be honored.

NO REQUESTS FOR REFUNDS WILL BE CONSIDERED AFTER 7 MARCH 2016.
